For patients who have difficulty attending regular daytime appointments and can only come in during evenings or on Saturdays, please call Dr. Mehdi’s office. You may be accommodated in one of Dr. Mehdi’s After-Hours Clinics. Please note that these appointments are limited and are reserved for those with genuine need.

To register as a new patient:
- Download and complete the Patient Registration Form.
- Email the completed form to drlaraibmehdi@gmail.com.
- Wait to be contacted to schedule your meet-and-greet appointment. Please allow approximately 3–4 weeks for an appointment to be arranged.
Note: Submitting an intake form does not confirm acceptance as a patient. Registration will be finalized after your meet-and-greet appointment.
- Health Advisory
If you have an acute viral illness that started within the last 24 hours or active gastroenteritis with vomiting/diarrhea, please do not come to the clinic, as it could be highly contagious. Instead, take care of yourself at home, monitor your symptoms, and call us to leave a message for triage.
